Provider Requests for Criteria
Questions or issues regarding Meritage Medical Network's authorization process, or how to obtain the
policies, procedures, and criteria used to authorize, modify or deny healthcare to contracted health care
practitioners or providers, can be obtained by calling the Utilization Review Department. Access to UM staff
for authorization issues or a physician reviewer to discuss denials of service is available during normal
business hours (8:30am - 5:00pm) at 415-884-1840 or toll free at 1-800-874-0840.
